About the APS Toolkit
The APS (Adult Protective Services) toolkit contains numerous policies, procedures, forms, and best practices templates for APS in Wisconsin with contributions from many county APS departments statewide, DHS (Department of Health Services), and GWAAR (Greater Wisconsin Agency on Aging Resources, Inc.). The contents of this toolkit are not “official” DHS forms. The contents are intended to be used and/or modified to meet the needs of an individual county and are not mandatory. This toolkit will be reviewed on a regular basis to ensure the content continues to be relevant. Please contact DHSAreaAdmin@dhs.wisconsin.gov with comments, questions or to submit additional documents to be considered for the APS toolkit.
Tips for Navigating the Library
This library contains over 200 documents. If you leave the “Search” box empty, every document in the toolkit will display.
The files are sorted broadly into seven categories. To narrow your search to just the documents in a certain category, type one of the categories into the “Search” box.

To narrow your search even further, you can search by key terms that show up in the document’s title. For example, if you enter “hoarding” into the “Search” box, all documents with hoarding in the title will display.
Note: The search function works best searching one word at-a-time. For the best results with your search, use singular words instead of plural words (e.g., search “closure” verses “closures”).
