ADRC: Help Making Long-Term Care Decisions for Older People and Adults with Disabilities
Aging and disability resource centers (ADRC) can help people decide whether to move from home. This service, called pre-admission consultation (PAC), helps older people and adults with disabilities—and the loved ones who help them—understand their long-term care options.
That may include:
- Moving into a nursing home or an assisted living facility.
- Staying at home with in-home services put in place.
By talking to an ADRC specialist, you can learn:
- Available options.
- If you qualify for government funding.
- The costs of your options.
Long-term care facilities must make prospective residents aware of the option for preadmissions counseling. This is a statutory requirement for nursing homes, assisted living facilities, community-based residential facilities, and residential care apartment complexes. The ADRC brochure Considering Long-Term Care. You Have Options, P-03712E (PDF) is available to facilities for this purpose. The brochure encourages people anywhere in the state to connect to their local ADRC or Tribal partner agency by calling 1-844-WIS-ADRC or visiting FindMyADRC.org.
The following list contains alternative versions of Considering Long-Term Care. You Have Options, P-03712E (PDF) with an editable field for the insertion of local ADRC or Tribal ADRS contact information. The brochure is available in Arabic, Chinese (Simplified), Hmong, Laotian, Russian, Serbian (Serbo-Croatian), Somali, and Spanish. Translation and interpreter services are also available by request from the local ADRC or Tribal ADRS.
