Employee and Contractor Background Check Process
Entities must complete background checks for all employees and contractors meeting the definition of a "caregiver" under Wis. Stat. § 50.065(1)(ag)1. These caregiver background checks are required at the time of hire, whenever there is a change in circumstances (arrest, conviction, government investigation, etc.) and at least every four years.
To obtain a caregiver background check online:
- The employee or contractor must complete a Background Information Disclosure (BID), F-82064.
- The entity must request a caregiver (note: this option must be selected) background check from the Wisconsin Department of Justice (DOJ) using the Wisconsin Online Record Check System (WORCS).
- Pay $15 per person fee to DOJ with a debit or credit card through WORCS.
Note: A log in to the WORCS system is not necessary, but providers can follow the steps outlined by DOJ to request a billing account.
If the entity is unable to submit an online request, complete the following steps:
- The employee or contractor must complete a Background Information Disclosure (BID), F-82064.
Review Wisconsin Department of Justice (DOJ), Background Check & Criminal History Information and complete a Wisconsin Criminal History Record Request by using the Single Name Record Request, Form DJ-LE-250 (PDF) form.
Mark "Caregiver-General" under Request Purpose. Mail completed form along with appropriate fee payable to "Department of Justice" and self-addressed stamped envelope to:

Obtaining additional information for background checks
In some circumstances, entities may be required to obtain additional information, such as judgments of conviction, criminal complaints, out-of-state background checks, and military discharge documents.
