 Basic dental: This plan covers the following each year:

  • Two oral exams
  • Two cleanings
  • One dental X-ray
  • One fluoride treatment

 Extra dental: Up to a $3,500 for covered comprehensive dental services

 Vision: Up to $450 for eyeglasses or contact lenses every year

 Hearing:

  • One routine hearing exam and hearing aid fitting/evaluation every year
  • Up to $3,000 for hearing aids

Wellness programs: Silver Sneakers

Other:

  • $150/month for over-the-counter drugs and/or groceries
  • 42 trips transportation
  • Podiatry
  • Essential Extras benefit
