ForwardHealth: Wisconsin Chronic Disease Program
The Wisconsin Chronic Disease Program (WCDP) helps people with chronic diseases get care. Chronic diseases are health conditions that are long-lasting. WCDP helps people get services and supplies that treat:
- Adult cystic fibrosis
- Hemophilia
- Renal Disease
The Wisconsin Department of Health Services (DHS) funds the program.
Program members must pay some copays and deductibles. If you make more than 300% of the federal poverty level each year, you must pay a certain amount yourself before getting benefits.
The state gets paid for benefits it provides through the Wisconsin Estate Recovery Program.
Chronic disease programs
The Chronic Renal Disease Program helps people with advanced renal disease.
Program members who can get Medicare must pay Medicare Part B premiums to get benefits. These members can get benefits the day they start treatment.
Program members who can get Medicare, but don’t, are not able to get benefits until they start paying premiums.
Program members can get these covered:
- Certain dialysis and transplant treatments
- Certain home supplies
- Certain lab and X-ray services
- Certain medicines
- One dental exam and X-rays
- Transplant services

The Hemophilia Home Care Program helps people with hemophilia. A hemophilia treatment center must have diagnosed them. They also must have agreed in writing to enter a treatment program through the center.
Members start getting benefits the day their application is received.
Program members can get these items covered:
- Blood products
- Home infusion supplies
They must pay a $10 copay for each medicine and blood product the program covers.

The Adult Cystic Fibrosis Program helps adults with cystic fibrosis. Members must be 18 years or older. The medical director of a cystic fibrosis center must have diagnosed them.
