Chronic Disease Prevention Program: Training
The Chronic Disease Prevention Team is happy to offer training opportunities to help improve the health of Wisconsinites.
Many trainings offered by State of Wisconsin agencies are currently housed on Cornerstone. Please see the "Access to Training" and "Frequently asked Questions" sections below for information on how to create and use a Cornerstone account. If you already have a Cornerstone account, click the link to your desired training below.
Self-Measured Blood Pressure Training for providers
Title: SMBP: Prepare and Teach Your Patient.
This training provides an overview for health care providers on self-measured blood pressure (SMBP). It includes strategies for engaging patient motivation, selecting a validated device, checking the accuracy of a device, and planning a tracking method. Two optional sections are also included to assist you: a short list of recommended resources for providers and a self-test to validate your understanding of the material covered in the provider lessons.
Health care providers should consider sharing the patient-focused training listed below with their patients to reinforce education provided in the clinic.
Self-Measured Blood Pressure Training for patients
Title: Taking Your Blood Pressure at Home
This training is geared towards patients. Your health care provider may have shared this training with you to help you learn more about measuring blood pressure at home. While you've likely learned a little bit about measuring blood pressure, this training will help reinforce and build upon what you have already learned. You will explore how to prepare to take a blood pressure reading (PDF) as well as how to track your readings using logs (PDF) or wallet cards, P-02102 (PDF). Make sure to check in with your health care provider on how they would like you to record and share your readings.
You can take this training multiple times. You could even use it each time you need to take a blood pressure reading.
Access to training
To access these courses, you must register to use Cornerstone, if you don't already have a Cornerstone account. Select the option below that applies to you.
- Create a Cornerstone account. Follow the prompts on each screen. The email address you enter on the first screen will be your Cornerstone username.
- Choose a password and security question answers you will remember as we will not be able to retrieve this information for you.
- When answering your security questions, you must enter the answer exactly as you typed it when you created your account. Answers are case sensitive. That means that, if you used capital (uppercase) letters when entering an answer when setting up your account, you need to capitalize those same letters when answering the questions to reset your password. If you entered a date as an answer, you will have to type the date exactly as you did during account setup, including using the same dashes, hyphens, and other characters.
- We do not have access to your password or any answers to security questions.
- Exit Cornerstone after you have created an account. Wait at least 15 minutes.
- Follow the steps listed below under "I have a Cornerstone account" to take the training.
The following are some common error messages you might be receiving.
Email address invalid
Email must be in correct email format (for example: @abc.com or @gmail.com). Double check your spelling and try again.
Email already in use
Reenter the email address and try again. If the error persists, it could mean that you have already used your email in a previous registration. If you already have a user account, log into Cornerstone. If you cannot remember your password, select the "Forgot Password?" option on the login screen to reset it.
User ID or username already in use
Reenter the username and try again. If the error persists, it could mean that you have already used your email in a previous registration. If you already have a user account, log into Cornerstone. If you cannot remember your password, select the "Forgot Password?" option on the login screen to reset it.
We are unable to retrieve your password. You must use the password reset function in Cornerstone.
- Go to Cornerstone.
- Select the "Forgot Password?" option on the login screen.
- Enter your username (the email address you used when you created your Cornerstone account) into the "Login Credential" field.
- Check the "I’m not a robot" box.
- Select the "Submit" button.
In order to receive the email with a link to reset your password, you will have to supply the answers to your security questions. Please note that you must enter the answer exactly as you typed it when you created your account. Answers to security questions are case sensitive. Every DHS employee, contractor or person of interest that needs equipment, a DHS email address or network access has a STAR Employee Self Service (ESS) account and therefore has access to Cornerstone through the “My Learning” link within STAR ESS. DHS employees, contractors or persons of interest with a STAR ESS account should not set up an external account to access Cornerstone.
If you have a DHS email address, do not set up an account through self-registration.
- Log into Cornerstone.
- Search for the training you're looking for. Help guide: Search for Training (PDF)
- Launch the training.
- Select the blue "Open Curriculum" button.
- Select the blue "Launch" button to begin the course.
NOTE: It may take a minute or two for the training to load. Be patient. If the training does not load in a few minutes, follow the prompt on the screen to open it.

Answers to frequently asked questions
We recommend that you use either Google Chrome or Microsoft Edge. Internet Explorer will not work.
No, you do not need to complete trainings in one sitting. In fact, you may pause trainings at any time.
If you exit a training completely, your progress will be saved. You will then be able to return to where you left off the next time you log in.
That will depend on the individual training. Please see the description for the training you'd like to take.

Not all offerings will have multiple sections. However, if a training has multiple sections, the second section will only become available after you've finished the first.
At the beginning of some trainings, you may be asked if you want a certificate of completion. Respond yes or no. If you complete all of the training, a certificate of completion will become available. It is not necessary to print or save these certificates as they will be accessible via your account.
