Nurse Aide Program: Provider FAQs

Wisconsin TestMaster Universe (TMU) houses the Wisconsin Nurse Aide Registry. Headmaster hosts the registry and administers our testing program.

How do I verify a nurse aide's status?

Log in to your facility's account on TMU. Use this online Wisconsin Nurse Aide Registry to check the person's status. As a regulated provider, you must verify they are eligible to work before hiring the person.

You can search by name, Social Security number, or registration number. The registry displays the aide's:

  • Name.
  • Inclusion date.
  • Expiration date for verifying eligibility to work in federally certified facilities.
  • Basis for inclusion.
  • Facilities where the aide is eligible to work.
  • Substantiated finding information (abuse or neglect of a client or misappropriation of a client's property) if any.

What if I have a potential employee who is inactive on the registry and needs to renew eligibility?

If a nurse aide has not worked in the previous 24 months, they are inactive. To renew their federal eligibility, they must pass the knowledge test and the skill test. Individuals interested in retesting should contact Headmaster toll-free at 888-401-0465. Headmaster will help them set up an account on TMU.

Can we use non-certified individuals for some services?

Yes. Regulations allow you, as the provider, to add tasks for unlicensed personnel. Contact us with questions about the scope of practice for unlicensed workers in a specific provider setting.

Refer to the Wisconsin Admin. Code ch. DHS 129.07 for the scope of practice for a nurse aide. The code lists the skills taught in all approved nurse aide training programs.
