- County Health Rankings & Roadmaps is a collaboration between the Robert Wood Johnson Foundation and the University of Wisconsin’s Population Health Institute. It gives annual measures for vital health factors by county. These include high school graduation rates, obesity, smoking, unemployment, access to healthy foods, the quality of air and water, income inequality, and teen births.
- Wisconsin Chronic Disease Quality Improvement Project is a joint effort between Wisconsin Health Plans, the Department of Health Services’ Chronic Disease Prevention Program, and the University of Wisconsin’s Population Health Institute. It publishes annual reports on the quality of chronic disease prevention and care.
- Wisconsin Cancer Facts and Figures reports data on the burden of cancer in Wisconsin.
- Environmental Public Health Tracking gives data on more than a dozen environmental public health topics. Some of these topics include oral health, asthma, cancer, and emergency department visits and/or hospitalizations for heart attack.
- Wisconsin Health Atlas is funded by the Wisconsin Partnership Program. Employees of the Obesity Prevention Initiative and the University of Wisconsin’s Population Health Institute staff the program. This resource provides a statewide dashboard on weight, healthy eating, and physical activity. It equips leaders at the local level to affect positive change by informing and supporting health promotion efforts.
- WISH (Wisconsin Interactive Statistics on Health) Query System houses state-level health indicators. They include health behaviors, hospitalizations, mortality, and morbidity data.
- Survey of the Health of Wisconsin is an ongoing, annual health survey that gathers data on health and a wide range of factors that determine a person’s health. Surveys are completed by physical measurement and bio sample collection. Those surveyed live in diverse urban and rural areas across the state.
- America’s Health Rankings provides historical and complete health, environmental, and socioeconomic data. It’s used to show national health benchmarks and state rankings.
- The CDC offers Prevalence Data and Data Analysis Tools on its Behavior Risk Factor Surveillance System. The tools allow you to find city and county data collected through various resources.
- PLACES: Local Data for Better Health provides estimates of health measures for all counties, places (incorporated and census designated places), census tracts, and ZIP Code Tabulation Areas (ZCTAs) across the United States.
