Templates for environmental risk assessment

Lead hazard investigators and lead risk assessors from local health departments are required to use the following templates for property investigations in cases of lead-poisoned children. If your health department is using a different template, you may submit it for review and approval by the Wisconsin Department of Health Services (DHS).

Wisconsin has a number of statutes and administrative rules with respect to childhood lead poisoning prevention, including those that pertain to local health departments, child care centers, and other facilities and departments.
