Templates for environmental risk assessment
Lead hazard investigators and lead risk assessors from local health departments are required to use the following templates for property investigations in cases of lead-poisoned children. If your health department is using a different template, you may submit it for review and approval by the Wisconsin Department of Health Services (DHS).
