Family Care: Long-Term Care Option for Tribal Members

Wisconsin Department of Health Services (DHS) values the freedom, culture, and unique needs of American Indian and Alaskan Native people. We offer inclusive services in Medicaid programs like Family Care.

Options for tribal members

We’ve worked with 11 tribes in Wisconsin to set up an option for tribal members to get care management from an Indian Health Care Provider. Members can choose to receive care management from an Indian Health Care Provider instead of the typical managed care organization. The tribe must have a signed three-party agreement with the managed care organization and DHS.

Care management

Two tribes have signed 3rd party agreements. These allow their members to get care management from an Indian Health Care Provider. Tribes include:

  • Menominee Indian Tribe of Wisconsin
  • Oneida Nation

Care management is a specific service given by nurses or social workers. It involves creating a care plan based on the member’s needs and outcomes. The plan defines what services will meet those needs and outcomes.

All other services

Family Care members from any tribe can get other covered services from out of network Indian Health Care Providers.
