Long-Term Care Data Warehouse Release Notes

Release notes are summaries of original releases and recent changes to Long-Term Care (LTCare) Data Warehouse universes, which are business representations of data. The release notes are intended as supplementary information about recent enhancements or bug fixes to the system.

Select a data mart universe below and then the release number to view the release notes. The most recent releases are listed first. All documents are provided in portable document format (PDF).

Data dictionaries describe the data in the universe. The dictionary lists the name used for the data, a definition or description of the data, and indicates all of the universes that have that data. Although they are not accessible on this page, if you need a data dictionary for one of the releases, contact DHSLTCareDataWarehouseHelp@wisconsin.gov.

The LTCare Adult Personal Care Functional Screen Datamart Universe houses Wisconsin's Functional Screen information about an individual's functional status, health, and need for assistance for various programs that serve the frail elderly and people with intellectual/developmental or physical disabilities. The information is used to determine functional eligibility for certain mental health services and adult long-term care programs.

The information is used to determine functional eligibility for certain mental health services and children's long-term support programs.

The Incident Tracking Data Mart has been created to provide access to Incident Tracking data submitted through the Information Exchange System's (IES) Encounter Reporting by Managed Care Organizations (MCOs) throughout Wisconsin. This data mart allows analysis to be performed on Incident Tracking data by both state and external employees. It is accessible through the ForwardHealth BusinessObjects website and requires specific access to be granted in order to log in and access the data.​
