IRIS Participant Toolkit for Adult Long-Term Care Provider Enrollment

Your providers are the people, organizations, and companies that do things you need help with, also called services, through the IRIS (Include, Respect, I Self-Direct) program. Providers are paid by Wisconsin Medicaid for IRIS services. Wisconsin Medicaid has changed how your providers sign up to be able to provide services to you:

  • Participant-hired workers (PHWs) do not need to sign up. They will keep working as usual.
  • All of your other providers must start signing up to be a Medicaid provider using the ForwardHealth Portal right away.

If your providers don’t start signing up now, they won’t get paid to deliver your IRIS services in 2026. Ask your providers if they are planning to sign up to provide Medicaid services in 2026. If they don’t, that means you’ll have to find new providers.

We need your help! You should:

  1. Check your IRIS Individual Services and Supports Plan (ISSP) to see what types of providers you have. Your participant-hired workers do not need to sign up.
  2. Tell your providers they need to start signing up right away. If they don’t, they won’t get paid to deliver your services in 2026.
    • Signing up and finalizing applications takes a few weeks.
    • Talk with your providers to make sure they know about the new way to sign up. Tell them how important it is to sign up right away.
  3. If you have already spoken with your provider and they said they do not plan to sign up, ask your IRIS consultant for help finding a new provider.
