EMS: Verification of Wisconsin Certification or License

If you are requesting the Wisconsin EMS Section to provide a verification of your current or expired Wisconsin certification or license, please complete the process stated in Wis. Admin. Code § DHS 110.16(1)(d)

Please provide the form from the other state, jurisdiction, or agency that is requesting verification of your Wisconsin certification or license by the Wisconsin EMS Section and send a cashier’s check or money order for $25.00 to:

Per Wis. Admin. Code § DHS 110.16(2), the Department of Health Services will not accept cash payment or personal checks. You must send a cashier’s check or money order. We cannot process the verification of Wisconsin certification or license document without the $25.00 fee. Fees are nonrefundable and nontransferable.

Once the Wisconsin EMS Section receives the verification of Wisconsin certification or license document and fee, we will complete the document. The document will be added to your Wisconsin E-licensing account and will be located in your “Document” tab for your records.

We will then email the verification of Wisconsin certification or license document to the identified recipient on the document. You will be added to the email communication being sent. A copy of the sent email will be added to your Wisconsin E-licensing account and will be located in your “Document” tab for your records.
