Diabetes: Professional Resources
The Wisconsin Department of Health Services (DHS) Chronic Disease Prevention Program invites you to team up with us to reduce the impact of diabetes in our state. Our goals include prevention and improved management of the disease. We offer resources to help with your efforts.
Medical care guidelines
The Wisconsin Diabetes Advisory Group recommends providers follow the Standards of Medical Care in Diabetes. These guidelines are set by the American Diabetes Association for high-quality, evidence-based clinical care. Highly trained clinicians and researchers base these guidelines on a complete review of current studies and publications. This ranges from rigorous double-blind clinical trials to expert opinions.
After weighing the quality of evidence, recommendations are drafted, reviewed, and submitted to the American Diabetes Association’s Executive Committee for approval. They are revised on a regular basis and published annually. The American Diabetes Association updates the Standards of Care throughout the year. Using the Standards of Care in Diabetes app provides the most up-to-date diabetes care recommendations. You can access the ADA Standards of Medical Care in Diabetes app.
Materials for your patients
- Diabetes Kickstart handouts complement the Diabetes Kickstart Video series from the CDC (Centers for Disease Control and Prevention). Digital and print-ready files are available to download in both English and Spanish.
- Diabetes Self-Care Guide, P-43081 (available in English, Spanish and Hmong for download). This 12-page booklet explains diabetes and self-care. It lists the tests, exams, and medical checks patients need to take care of their diabetes.
- Blood Glucose (Sugar) Log Booklet, P-00246 is available in English and Spanish for download.
