In-Home Care

In-home care service providers accommodate a range of needs that enables adults living with a disability and aging adults to remain independent, maintain a healthy quality of life, and continue living at home. Services provided may include personal care, supportive care, and companionship care. The goal of an in-home service provider is to ensure that individuals feel comfortable, safe, and well-cared for in the comfort of their own homes.   • Case Management

    Provides support finding the right care and services to support an individuals needs.

  • Companionship Care

    Provides emotional support and socialization through companionship.

  • Hospice

    Provides care to those with a terminal illness, approaching the end of life.

  • Medical Home Health Care

    Medical home health care offers medical services to individuals in their own home or place of residence.

  • Medication Management

    Medication management provides assistance ensuring that medications are administered and taken as directed.

  • Personal Care

    Provides support and supervision of daily personal living tasks and hygiene.

  • Respite Care

    Respite care is a short-term program that provides relief for caregivers by allowing them to take a break from their caregiving responsibilities.

  • Supportive Care

    Provides assistance completing household tasks or chores.
