Radioactive Materials: General Licenses - Authorization and Registration

Authorization

General licenses are issued in Wis. Admin. Code § DHS 157.11 and Wis. Admin. Code § DHS 157.93 authorizing the receipt, possession, use, or transfer of certain radioactive materials and devices containing radioactive materials. A person does not need to apply for a general license.

Registration

Radioactive materials (in isotopes and quantities listed below) that are contained in devices under general license authorization must be annually registered with the Department of Health Services (DHS). Registration is for the calendar year. Registration requirements and annual fees are listed in Wis. Admin. Code § DHS 157.12.

*Definition: Transuranic - Having an atomic number greater than 92.

Inspection

DHS will provide a self-inspection form, F-45021 (Word) with each annual registration fee bill. General licensees will be asked to complete the form, F-45021, and return it to DHS. Failure to complete and return the form may result in a site inspection.

Annual fee

A general licensee shall pay an annual registration fee of $100 per site and $50 per device.

DHS may not assess an additional fee for recording changes in registration information.

The annual registration fee for the next year shall be paid by December 31 of the prior year of registration. 

DHS shall issue a notice of registration following receipt of the registration fee. If the annual registration fee for the next year is not received by DHS by December 31 of the prior year of registration, a license shall pay a penalty fee of $25, in addition to the registration fee and regardless of the number of devices, before DHS will issue a new notice of registration.
