Provider post survey questionnaire
DQA offers a Post Survey Questionnaire for health care providers to complete after a survey experience.
Regulation for clinics
DQA doesn't regulate free-standing outpatient clinics and urgent care centers unless they are under a hospital's Medicare number. If you are opening a clinic, please work with your local municipality and/or the Department of Safety and Professional Services. They can help you with any building or clinician professional licensure and credentialing.
