Immunizations: Yellow Fever Vaccination Center Certification
To offer the yellow fever vaccine to the public, you must be authorized to use an official yellow fever stamp. There is one uniform stamp used by all Yellow Fever Vaccination Centers. The State of Wisconsin authorizes use of the stamp in our state.
The uniform stamp is used on the patient’s International Certificate of Vaccination or Prophylaxis (ICVP) card. It verifies that the person has been vaccinated against yellow fever.
The yellow fever vaccine has a risk of serious side effects. Providers only should vaccinate people who:
- Are at risk for exposure to yellow fever virus.
- Require proof of vaccination for entry into a country. To further reduce the risk, health care providers should:
- Carefully observe contraindications.
- Consider the precautions to giving the yellow fever vaccine.
Requirements to become a yellow fever uniform stamp holder
- Applicants must be a physician (medical doctor or doctor of osteopathic medicine), physician assistant, or nurse practitioner with an active license in the State of Wisconsin.
- Read and understand the Center for Disease Control and Prevention’s Yellow Fever Vaccine: Recommendations of the Advisory Committee on Immunization Practices (ACIP).
- Agree to record each dose of yellow fever vaccine administered in the Wisconsin Immunization Registry. If you aren’t a registry user, please email the Wisconsin Immunization Registry help desk at dhswirhelp@dhs.wisconsin.gov or call 608-266-9691. If you use the Registry for Effectively Communicating Immunization Needs (RECIN), you don’t need to use the Wisconsin Immunization Registry.
- Purchase an official yellow fever uniform stamp for each official Wisconsin Yellow Fever Vaccination Center.
- Give the Vaccine Information Statement to every yellow fever vaccine recipient before administering the vaccine.
- Adhere to administration, storage, and handling requirements as indicated by Advisory Committee on Immunization Practices and the yellow fever vaccine manufacturer.
- Administer yellow fever vaccines only at an official Wisconsin Yellow Fever Vaccination Center.
- Record yellow fever vaccines with official uniform stamp on the ICVP card.
- Re-certify as a Yellow Fever Vaccination Center every three years. The manufacturer won’t ship yellow fever vaccines after your official yellow fever uniform stamp expires.
- Notify the Wisconsin Immunization Program if the official uniform stamp holder leaves. Also notify us if there’s an address change for the Yellow Fever Vaccination Center.
- Each provider may register a maximum of five yellow fever centers.
Steps to become or re-certify as a Wisconsin Yellow Fever Uniform Stamp Holder
- Complete the Centers for Disease Control and Prevention’s online training Yellow Fever Vaccine Course. Any staff advising travelers on behalf of the stamp holder also should complete the course.
- At the end of the course, save the Certification of Completion, which includes the training date. The applicant must upload the certification into the application in step 3.
- Complete the Yellow Fever Vaccine Certification application. You can complete this application over several sessions. Keep track of your survey number, since that information is available only to you. To return to the application, submit your return code given when you exited the application. This step allows you to return to the application you began in an earlier session. Wisconsin Immunization Program staff doesn’t have access to your survey number. We can’t help you if the number is lost or forgotten.
- While you wait for your certification application to be processed, ensure you have an active account with the yellow fever vaccination (YF VAX) manufacturer Sanofi Pasteur. Call 1-800-VACCINE (1-800-822-2463).
