Consumer Guide: Problems with Clinical Laboratories including Facilities that Commonly Perform Waived Testing for Clients

The Clinical Laboratory Improvement Amendments (CLIA) section oversees CLIA laboratories in Wisconsin on behalf of the federal CLIA program administered by the Centers for Medicare & Medicaid Services (CMS). The CLIA section is part of the Division of Quality Assurance (DQA) which is in charge of protecting the health, safety, and welfare of people who use health and residential care services in Wisconsin.

Besides hospital and clinic laboratories, other examples of CLIA certified laboratories include service providers that perform laboratory testing in residential care facilities, school health offices, plasma centers, pharmacies, ambulance services, and public health offices. Common complaints about laboratory operations include issues about the quality of testing, missing or incorrect test results, unlabeled specimens, record falsification, confidentiality of patient information or personnel qualifications and service.

Complaints may be filed anonymously. If you provide your contact information, everything will be done to protect the privacy and anonymity of your complaint as permitted by Federal and State laws.

File a complaint against a CLIA certified clinical laboratory

There are several ways to file a complaint against a CLIA certified clinical laboratory:

  • File a complaint online by completing the Complaint Intake Survey for CLIA Laboratories, F-03352.
  • Call the toll-free number to file a complaint by dialing 800-642-6552. Leave a voicemail message with your phone number and the name of the laboratory. Your call will be returned by the next working day.
