Care4Kids Program: Program Benefits

Care4Kids creates a "medical home" for each child entering out-of-home care. This "medical home" is not a physical place, but rather a philosophy that children in out-of-home care (OHC) deserve coordinated and comprehensive healthcare that addresses their unique needs. Care4Kids helps children and teens in foster care find the health services they need, when they need them.

Find providers

Care4Kids uses Chorus Community Health Plan to provide services. You can pick any doctor or provider in the Plan for your child.
