Family Health Survey: Information for Researchers
The Family Health Survey questionnaire is administered over telephone by trained interviewers. The adult household member who self-identifies to know the most about the health of all household members responds for the whole household. Currently, the questionnaire includes the following modules:

- Health care utilization
- Health literacy
- Health conditions
- Health insurance
- Employment
- Demographics
- Income

Request Family Health Survey data
If you would like to request summary data tables on one of the topics that Family Health Survey collects, send an email to DHSFHS@dhs.wisconsin.gov with the details of your request.
If you would like access to Family Health Survey data to conduct your own analysis using appropriate statistical software, review the Technical Notes, P-45369B (03/24) (PDF), document and the relevant questionnaire and data dictionary above, and email us at DHSFHS@dhs.wisconsin.gov to get a data application. Upon approval, you will sign a Data Use Agreement and receive access to the data you requested. You will be responsible for storing the data securely and using appropriate analysis methods.
