Level I screens
Level I screens must be submitted through the ForwardHealth Portal.

Level II screens
Level I screens submitted through the ForwardHealth Portal identified for referral for a Level II screen will automatically be sent to Behavioral Consulting Services for review.
Behavioral Consulting Services is Wisconsin's PASRR contractor. To contact Behavioral Consulting Services, call 262-376-1223 or email info@behavioralcs.com (do not send protected health information to this email address).
Short-term exemptions
For nursing facilities
- Nursing facilities must request county approval for all short-term exemptions using the County Review of Nursing Home, IMD, or ICF/IID Referrals, F-20822. The county liaison must fill out, sign, and date F-20822.
- Requests for F-20822 must be made prior to admission.
- Requests for F-20822 should be made by completing a Level I screen for the resident, and then printing and faxing the Level I to the PASRR county liaison.
- Once the signed F-20822 form is received, nursing facilities should upload the F-20822 to the individual’s specific PASRR ID within the ForwardHealth Level I Wizard to show the short-term exemption has been approved.
- If it is determined that the individual (who is suspected of having a mental illness and/or a developmental disability) will need to stay beyond the short-term exemption period, a Level II screen must be completed. Nursing facilities should update the individual’s existing Level I screen in the ForwardHealth Level I Wizard to indicate “Yes” to “Is the person staying longer than the permitted exemption period?” and upload the requested clinical documentation.
For county liaisons
- Nursing facilities should fax requests for approval for short-term exemptions.
- Requests use the County Review of Nursing Home, IMD, or ICF/IID Referrals, F-20822, which is filled out by the county liaisons.
- PASRR county liaisons should sign and date F-20822.
- F-20822 should be returned to nursing facilities by fax in a timely manner.
For out-of-state requests
Nursing facilities seeking 30-day recuperative care short-term exemptions for out-of-state residents can request approval by faxing the PASRR 30-Day Information Required for PASRR Exemption Letter Request, F-02784, to 608-267-7793. This form does not apply to in-state residents.

The Center of Excellence for Behavioral Health in Nursing Facilities provides support to skilled nursing facilities at every level, from the nursing staff who provide direct care to residents, to the clinical and administrative staff who interact with them.
National Association of PASRR Professionals
The National Association of PASRR Professionals seeks to improve the quality of care for individuals with mental illness or intellectual/development disabilities who apply for admission or continued residence in nursing facilities.
