Children’s Community Options Program: Information for Counties
The Children’s Community Options Program (CCOP) is for families who need help caring for a child with long-term support needs. CCOP provides flexible funding and coordinates supports and services. These help the family care for their child at home and in community settings.
The family works with a local support and service coordinator from a county health and human services agency. The coordinator completes an assessment. It helps them understand the child’s and family’s abilities, strengths, needs, and desired outcomes. The assessment also looks at how the family wants to get services, where they live, medical care, and more.
From the assessment, the family and support and service coordinator create a personal service plan.
