Note: Multiple mailings occur throughout the year. Therefore, the date on your letter may be different than the letter on this webpage.

The Wisconsin Department of Health Services (DHS) is currently conducting the National Core Indicators Aging and Disabilities Survey (NCI-AD) for older adults and adults with physical disabilities. This survey is part of a nationwide project called National Core Indicators. DHS is doing the survey to learn more about how to help people in Wisconsin who are getting long-term care. The survey information will be used to improve services and better understand people’s needs.
