Functional Screen: Mental Health/Substance Use - Screener Instructions
Wisconsin’s Mental Health and Substance Use Disorder Functional Eligibility Screen was developed in 2001. It is a secure online functional needs assessment with programmed logic to determine eligibility for the following programs:
The screen determines eligibility for mental health and substance use disorder programs for people age 18 and over. It can be completed for youth age 16 and over to allow for advanced planning.
