COVID-19: Your Vaccination Record
It is important to keep track of your COVID-19 vaccination record. Your employer, school, or other organizations may ask for proof of your COVID-19 vaccination, or a copy of your vaccination record.
There are a few ways to show proof of COVID-19 vaccination.
Your COVID-19 vaccination card
You will receive a vaccination card when you get your first dose of mRNA COVID-19 vaccine or when you get your single dose of Johnson & Johnson COVID-19 vaccination. Your COVID-19 vaccination card has information on when and where you received your vaccine as well as other helpful information.
- Keep your card in a safe place.
- Take a picture of your card.
- Protect your health information. Do not post photos of your vaccination card online.
- Do not laminate your vaccination card.
If you lose your COVID-19 vaccination card, call the provider or organization where you were vaccinated. They may be able to give you a new vaccination card. The Wisconsin Department of Health Services (DHS) cannot issue COVID-19 vaccination cards.
Electronic Vaccination Records
If you received your vaccination in Wisconsin, an electronic copy of your vaccination record is available on the Wisconsin Immunization Registry (WIR). You can access WIR through any web browser and save the electronic copy to your computer or mobile device. Parents and caregivers can also search vaccination records for their children or dependents under 18.
Step 1 - Follow the links below to access the WIR immunization records portal.

Step 2 - On the next screen, enter the first name in the First Name field and last name in the Last Name field.
Step 3 - Enter the birth date using the MM/DD/YYYY format in the Birth Date field, or use the calendar by clicking the calendar icon to the right of the field.
Step 4 - Enter one of the following:
- Social Security number
- Medicaid ID
- Health Care member ID
If you do not know any of the information above, call the provider or organization where you got your COVID-19 vaccine. They may be able to give you your health care member ID number.
Step 5 - Click the Search button.
The next thing you will see is the vaccination record. Your COVID-19 vaccine record should be included in this list. If you do not see your vaccination record, or you receive an error message, call the provider or the organization where you got your COVID-19 vaccine.
Step 6 - Click print.
Click print to download a formal copy of your record which can be saved to your computer or mobile device, or printed. You can provide the printed record as proof of vaccination for child care, summer camp, school, university, or employment purposes.
Unable to locate your vaccination record in WIR?
If you are unable to locate your vaccine record in WIR or do not have the information needed to search, fill out the Wisconsin Immunization Registry Record Release Authorization, F-02487 (PDF) and have your records sent to you.
Got your COVID-19 vaccine in another state?
If you received your vaccination in another state, go to that state department of health’s website to search their vaccine registry.
