Maternal and Child Health Summit Provider Funding Application

The agency selected for this funding opportunity will be responsible for planning, executing, and facilitating two virtual Maternal and Child Health Summits in 2025 for Title V grantees (which includes Local and Tribal Health Agencies) that addresses various maternal and child health topics. 

This project begins January 1, 2025, and ends December 31, 2025. This contract will have a duration of one calendar year.

Grant funding is expressly dependent upon the availability of funds to the Wisconsin Department of Health Services from the Health Resources and Services Administration, Title V Maternal and Child Health Grant. Applicants will be awarded based on available funding, capacity, and ability to meet application criteria.

The survey will close on December 30, 2024, at 5 p.m. CT.

Title V background

The Wisconsin DHS Title V program provides funding and support to local and Tribal health agencies and other organizations that support the well-being of mothers, children, adolescents, and families. The current priorities of Wisconsin’s Title V program are to:

  • Advance equity and racial justice.
  • Assure access to quality health services.
  • Cultivate supportive social connections and community environments.
  • Enhance identification, access, and support for individuals with special health care needs and their families.
  • Improve perinatal outcomes.
  • Foster positive mental health and associated factors.
  • Promote optimal nutrition and physical activity.
