How to Order Forms and Publications

To order forms, brochures and other informational materials, select items from the publications directory. (PDF, 36 KB) 

You have the choice of placing your order in electronic or handwritten format. The DHS Forms Center will ship your materials via UPS within 7-10 working days (often much faster). 

Ordering Electronically 

To order by e-mail, download the order form.  

When you open the form, you may see a dialog box. If so, choose "Enable Macros" and then choose "Yes" when asked if the document is from a trusted source. 

Save the form on your computer for future use. Type your order on the form, then e-mail it as an attachment to: DHSfmDPHwic@wisconsin.gov

Ordering by e-mail is the fastest and most efficient way to obtain publications. In addition, you will receive an e-mail confirming your order and providing information about publications that are new or out of stock.

Handwritten Orders

To submit a handwritten order via US mail, use form F-80025 (formerly numbered as DMT 25). To obtain a supply of this form, call (608) 267-2215 or request it by e-mail.

Who Can Order?

These instructions apply to forms and publications distributed by the Bureau of Community Health Programs, including:

  • Women, Infants and Children Nutrition Program (WIC);
  • Farmers' Market Nutrition Program (FMNP); 
  • Family Health programs; and 
  • Maternal and Child Health programs.

To obtain publications from other programs, consult the Department's web page on ordering.

The system works most efficiently when you use the correct order form and publication numbers.
