System (infrastructure) priority: Community health improvement
processes and plans (CHIP)
Objective: By 2010, 100 percent of local health departments will
have implemented and evaluated a community health improvement plan that is
linked to the State Health Plan.

Limitations/Comments
There is a limited amount of literature on evidence-based practices for
the implementation and evaluation of community health improvement plans
linked to State Health Plans. Because of this, interventions to encourage community health improvement plans have been categorized
under "insufficient evidence to determine effectiveness." The interventions listed above are model frameworks and programs,
based on the consensus of expert panels at the Institute of Medicine, the
National Association of County and City Health Officials (NACCHO) or the
Public Health Practice Program Office, Centers for Disease Control and
Prevention (CDC). They are provided as resources for local health
departments that want to enhance their community health improvement
processes.
Evidence-based methodologies are designed to provide explicit links
between public health or clinical practice recommendations and scientific
evidence that demonstrates effectiveness. These methods are not
appropriate tests for public policies such as Wisconsin Statutes 250 and
251, whose mandates match the goals of a State Health Plan objective.
