Tracking Health Conditions
The Wisconsin Turning Point Initiative is a statewide policy and
planning process to transform Wisconsin's public health system for
the 21st century. One key activity was to initiate a data-guided process
that involved stakeholders to identify priority health conditions. The
process identified the priority health
conditions (PDF, 815 KB), which became the foundation
for the Wisconsin State Health Plan, Healthiest Wisconsin 2010.
This site provides population-based data for the priority health
conditions on the county, regional and state level. Many of the
conditions are measured by mortality data; however, some links (denoted
with an asterisk *) redirect the user to external sites that provide
other kinds of data. Future site updates will incorporate additional
measures from a variety of sources.
