Pearson VUE Registry Form (PDF, 198 KB) - Instructions:

1. Fill out form completely.
2. Student must sign the form. For Challenge Exam, make sure student signs at the time of the test so that you do not have to seek out the signature later.
3. Send form directly to Promissor (address provided on form).
