With the promulgation of Wisconsin Administrative code DHS 83 effective April 1, 2009 there will be changes with the department approved training programs.  Effective March 1, 2009, the department will no longer  be accepting applications to review training programs.

Approvals issued before April 1, 2009 shall expire on April 1, 2010 when the new approval system is implemented.  Providers should continue to send their staff to existing programs until the third party to approve training is secured.

Training programs are in alphabetical order by location (city), including non-Wisconsin locations (Minnesota, Oregon, Illinois, etc.).

  • All CBRF Training Programs (PDF, 191 KB) - both those training any applicant and those training their own staff only.  Alphabetical by community name.  Includes training program number for verification purposes.

The following list only programs that provide training for any applicant. Those who provide training only for their own staff are not included.

Training Certificates:

A person who successfully completes an approved training program is considered to have met the training requirement. Approved training is "portable" to other CBRFs that serve the same client group(s).

To ensure the "portability" of training, students who complete approved training must have copies of certificates to provide to prospective employers. A certificate of completion must include:

  • The name of the agency offering the approved training

  • The title of the training module

  • The name of the student

  • The signature of the instructor

  • The Department approval number

  • The date of completion of the program

If you are encountering problems with approved trainers failing to provide certificates, or offering incomplete information on certificates, please contact the Licensing Policy Coordinator at (608) 266-7463 or E-mail the Division of Quality Assurance, attention Bureau of Assisted Living.
