Select Licensing and Permitting listed under Quick
Links (on the right-hand side of the page).

Go to the "Search by State Agency here"
area.

Select "Department of Health Services" from
the drop-down list.

Select the Search button.
This will display license and certification data for DHS programs. You
will find information on what licenses or certificate types are available,
including, but not limited to:
- probationary/continuing licenses or certificates
- third-party payment (Medicare)
- relevant fees
- links to the appropriate DHS (DQA) Licensing pages
- telephone and E-mail contacts
