This clinical practice guideline (PDF,
273 KB) was developed to assist providers to improve care and reduce
paperwork by streamlining the care planning and documentation process. Nursing home
providers and the Bureau of Quality Assurance (BQA) collaboratively developed this
guideline but the concepts and resources will be helpful to other providers who wish to
streamline documentation and improve care. This guideline was successfully piloted in four
Wisconsin nursing homes.
Providers may incorporate the guideline into their policies and procedures for clinical
practice. This guideline is not a regulatory requirement; however, it is
consistent with federal and state regulations if properly implemented. It is designed to
give clinical care teams a tool for quality improvement. If a person’s clinical
condition is accurately assessed, and the concepts in the guideline are properly
implemented, conflict with state and federal regulations should not occur.

The providers who piloted the guideline gave a presentation about their
piloting experience; these presentations are available on videotape. This video is useful
for trainers to view before using the guideline, but it is not a training video that would
