This page provides access to detailed information about each nursing home and
each facility for the developmentally disabled in Wisconsin. The
information is drawn from responses to the 2000 Annual Survey of Nursing
Homes, and represents each facility's pages from the Wisconsin Nursing
Home Directory, 2000.
Data contained in each profile include the following:
1. A description of the facility's characteristics,
such as licensure,
ownership, Title 19 (Medicaid) certification and
services provided.
2. Measures of the facility's utilization, such as admissions,
discharges,
average daily census, and number of residents.
3. The percentage distribution of resident characteristics (age,
sex,
length of stay, level of care, diagnosis,
conditions and therapies).
4. A comparison of the facility to its peer groups.
Facility profiles are organized according to county.
