Services
PACT
Services include both direct and indirect services. The majority of
services are administered in the community. Services are on-going, and not
time limited, but vary in frequency and intensity based on individual
client needs. Program staff is available 24 hours per day, seven days per
week, with program hours from 8:00 a.m. until 10:00 p.m., Monday through
Friday, and 10:00 a.m. until 6:00 p.m. on weekends and holidays.
Outside of regular office hours, staff is available by phone or in person
through an on-call system.
Direct
Services provided by PACT include:
- Prescription, acquisition,
set-up, and distribution of oral and IM medications. Monitoring of
efficacy and side effects
- Assessment of psychiatric
symptoms and status
- Supportive Psychotherapy
to address on-going needs as identified in the clinical treatment
plan and emergent needs
- Vocational assessment,
employment skills training, job development, job placement, job
coaching, on-going monitoring of job performance with client and
employer, and coordination of benefits, scheduling, etc. with
employer
- Educational assessment,
advocacy and participation in IEP assessment and implementation,
as well as on-going school conferences, coordination with special
education staff, arranging for tutoring, transportation to school,
provision of pre-employment skills groups
- ADL assessment (initial
and on-going), provision of training in self care skills, side by
side coaching and assistance with ADL tasks, accompaniment to
service appointments (e.g., doctors, dentist, agency meetings),
coaching on grooming, hygiene and general self care, assistance
with budgeting
- Assistance with housing
search and coordination and assistance with moving
- Assessment of physical
health care status and needs, assistance with securing health
care, transportation and accompaniment to appointments to
interpret medical information and to provide health history,
monitor and assist with follow through of prescribed health
therapy, ordering of needed lab tests and accompaniment to lab
appointments
- Social skills assessment
and training, providing group experiences for on-going social
skills practice and peer support, development and monitoring of
recreational activities
- Assessment of family
interaction and provision of family counseling as needed
- AODA assessment,
monitoring, and counseling
- Assessment and teaching of
parenting skills, education in child development, child discipline
and child safety, development of respite resources for the child,
recommendations to courts for child placement
- Crisis intervention,
including 24 hour on-call availability by phone and/or in person
- Provision of PM and
weekend support and structure
PACT
also provides the following Indirect Services:
- Comprehensive case
management of resources system wide (educational, vocational,
income and benefits coordination, health care coordination with
clinics and pharmacies, procuring specialized mental health
treatment services, ADL needs, housing programs, AODA services,
securing payeeships, legal advocacy, etc)
- Comprehensive Assessment
and Treatment Planning
- Provision of information
and technical assistance to employers, educators, the legal system
and family members
- Assessment of work
capacity, Disability Status, eligibility for benefits (social
security, housing subsidies, Medicaid, COP) and assistance with
securing needed benefits
- Advocacy within the school
and legal system for children of clients
- Coordination with legal
system on probation requirements and compliance
