Important information regarding provider certification!
Providers may obtain provider certification packets by downloading and printing them from this Web site or by contacting Wisconsin Medicaid Provider Services at (800) 947-9627 or (608) 221-9883.
The certification packets on this Web site are considered unsolicited provider certification packets (packets that are not requested in advance from Provider Services).
A provider will receive a certification effective date based on the date Wisconsin Medicaid receives a properly completed provider certification packet as long as the provider:
Meets all applicable licensure/certification requirements on the day of notification.

Returns a properly completed certification packet to Wisconsin Medicaid.
If the application needs to be returned to the provider for any reason, the provider will be given one 30-day extension. If the provider returns the completed materials within 30 days of the return day, he or she will receive the initial received date as his or her effective date of certification.
Providers that prefer to download the appropriate certification packet from this Web site should be prepared for longer downloading times compared to other publications on the Wisconsin Medicaid Web site. This list on this page includes the size of the Portable Document (PDF) file next to the particular provider type. Please use the chart below to determine how long it may take to
download the appropriate packet.
We recommend that you save the PDF to your hard drive of your computer once the PDF has opened in your browser. Please
remember to print the certification packet in its entirety.
