    On June 29, 2009, the Department of Health Services (DHS) issued a request for proposals from entities seeking certification by the Department to contract as a managed care organization for the delivery of long-term care to elders and adults with disabilities residing in Fond du Lac, Manitowoc, Winnebago, Calumet, Outagamie and Waupaca Counties.
