Wisconsin Diabetes Quality
Improvement Project
The Diabetes Quality Improvement
Project – called DQIP for short – is an extensive collaborative effort
of Wisconsin’s Community, Migrant and Homeless Health Centers to provide
the highest quality of care to people with diabetes.
Wisconsin Primary Health Care
Association (exit DHS) coordinates and provides technical assistance
to the DQIP for Wisconsin Community Health Centers with funding provided by
the Wisconsin Diabetes Prevention and Control Program (DPCP).
The Community Health Centers use a
team approach to provide diabetes care that is population-based,
evidence-based and patient-centered. Since 1998, 24 Health Center
sites have implemented the Wisconsin Essential Diabetes Mellitus Care
Guidelines, the Patient Electronic Care System (PECS) patient registry, and
ongoing quality improvement practices to continually improve care.
