Are You at Risk?

Type 2 diabetes prevention is possible, powerful, and proven!  It is important to find out early if you are at risk for Type 2 diabetes or if you have pre-diabetes.  To find out your risk, take the American Diabetes Association Risk Test (exit DHS).

This 12-page booklet explains diabetes, diabetes self-management, and lists the tests, exams, and medical checks you need to have in order to take care of your diabetes.

Take Charge of Your Diabetes (exit DHS)
This web-based resource was developed by the Centers for Disease Control and Prevention to help the person with diabetes take charge of the disease.  The resource is also available as a 148-page pdf document on the same website.

Diabetes and Heart Disease

Diabetes increases a person's risk for heart disease.  About three-quarters of people with diabetes die of some form of heart or blood vessel disease.  For people with diabetes, it is important to work with a healthcare provider to help in managing it and controlling other risk factors.  Learn more about diabetes and heart disease, click on the following links:

This National Diabetes Education Program (NDEP) developed this guide to educate and inform school personnel about diabetes, how it is managed, and how each member of the school staff can help meet the needs of students with the disease.

This educational DVD, available in English and Spanish, emphasizes the importance of knowing your risk of chronic kidney disease (CKD) and what you can do to prevent it.  After clicking on the above link, scroll down until you see "Watch the Chronic Kidney Disease DVD."  Links to an order form and a survey are also available on this website.  (English - 14 minutes, Spanish - 18 minutes)

Protect Your Vision: The Dilated Eye Exam (exit DHS)
This educational DVD, available in English and Spanish, emphasizes the importance of getting a dilated eye exam each year to prevent diabetic eye disease.  After clicking on the above link, scroll down until you see "Watch the Dilated Eye DVD."  A link to an order form is also available on this website.  (English - 9 minutes, Spanish - 10 minutes)
