Coordinated Care in a Medical Home

Children with special health care needs often require more medical care,
mental health services or education services than other children. They may be
enrolled in multiple state and federal programs for their specific needs,
including Medicaid, SCHIP, Social Security Income, early intervention and more.
As a result of complex conditions and many providers, families may need help
coordinating this care.
The CYSHCN Programs work to assure that children have a primary care provider
who serves as a Medical Home, providing comprehensive, family-centered care and
helping to coordinate the families’ medical and non-medical needs within their
community. The Regional Centers provide support and technical assistance to
families and providers in strategies to strengthen Medical Homes. The CYSHCN
Program and its Regional Centers have supported Medical Home learning
collaboratives throughout the state.
Building a Medical Home Partnership: A Wisconsin Toolkit (www.wimedicalhometoolkit.aap.org)
is a Web-based resource for Wisconsin healthcare providers interested in
learning more about family-centered care. The Toolkit contains tested tools and
practice strategies recommended by Medical Home teams both nationally and here
in Wisconsin.
For more information about Medical Home and to obtain an introductory packet
on the Wisconsin Toolkit, contact your Regional Children and Youth with Special
Health Care Needs Center. An Introductory Toolkit packet containing a CD-Rom
with four of the videos is available in limited quantities. The two Medical
Home brochures - Tips for Providers and Tips for Families can
be requested through your Regional Center.
