Wisconsin Collaborative for Cardiovascular Risk Reduction Initiative
The Wisconsin Cardiovascular Health Program, in partnership with multiple
organizations, is launching the statewide Collaborative for Cardiovascular
Risk Reduction Initiative. The initiative's goal is to control and / or
prevent cardiovascular disease throughout the state of Wisconsin. Over 7,000
health care providers across Wisconsin have been invited to participate in
promoting cardiovascular disease control and prevention.
Many of the risk factors of cardiovascular disease such as high blood
pressure, high cholesterol, and obesity are preventable and
modifiable. The Cardiovascular Risk Reduction Initiative promotes proactive risk factor assessment, testing, lifestyle modification
counseling, and appropriate medical treatment through a variety of tools and
materials.
The goals of this Initiative are to:
- Increase the rate of blood pressure screening
- Increase the rate of blood lipid profile testing
- Increase the rate of proper medical treatment
- Decrease the rate of cardiovascular disease risk factors
- Decrease the rate of cardiovascular disease events
- Decrease the deaths of cardiovascular disease in Wisconsin
- Improve the quality of care for cardiovascular disease patients
- Improve the quality of life for people with cardiovascular disease
risk factors in Wisconsin
- Promote healthy lifestyles
- Promote physical activity

Cardiovascular Disease

This is a laminated, easy to use, one-page (two-sides) provider
guidance tool, that incorporates recommendations from the American Heart
Association, the American College of Cardiologists, the National
Cholesterol Education Program
(ATP III), JNC 7, and other scientific statements and guidelines. New
recommendations from ATP III (PDF, 85 KB) are available in July 2004.

Risk Reduction Communication Record (PDF,
9 KB)
This form can be used to convey essential
cardiovascular-related lab test results, treatment and follow-up
recommendations to the patient’s health care providers.
- Personal
Heart Care Record (PDF, 141 KB)
Patients can use the wallet card to track their test dates, test
results, goals, and medications. These cards can be ordered from the Wisconsin
Cardiovascular
Health Program or can be personalized with your health system's name
/ logo.
You may request, at no-cost to you, these tools and materials by mailing
or faxing the Cardiovascular
Resource Materials Order Form (PDF, 69 KB)
to the Wisconsin Cardiovascular
Health Program. The order form also includes website locations for
guidelines and scientific statements that are referenced in the guidance
tool.
