Wisconsin Collaborative for Cardiovascular Risk Reduction Initiative

The Wisconsin Cardiovascular Health Program, in partnership with multiple organizations, is launching the statewide Collaborative for Cardiovascular Risk Reduction Initiative. The initiative's goal is to control and / or prevent cardiovascular disease throughout the state of Wisconsin. Over 7,000 health care providers across Wisconsin have been invited to participate in promoting cardiovascular disease control and prevention.

Many of the risk factors of cardiovascular disease such as high blood pressure, high cholesterol, and obesity are preventable and modifiable.   The Cardiovascular Risk Reduction Initiative promotes proactive risk factor assessment, testing, lifestyle modification counseling, and appropriate medical treatment through a variety of tools and materials.

The goals of this Initiative are to:

  • Increase the rate of blood pressure screening
  • Increase the rate of blood lipid profile testing
  • Increase the rate of proper medical treatment
  • Decrease the rate of cardiovascular disease risk factors
  • Decrease the rate of cardiovascular disease events
  • Decrease the deaths of cardiovascular disease in Wisconsin
  • Improve the quality of care for cardiovascular disease patients
  • Improve the quality of life for people with cardiovascular disease risk factors in Wisconsin
  • Promote healthy lifestyles
  • Promote physical activity

The Collaborative Cardiovascular Risk Reduction Initiative includes the following promotional tools and materials:

You may request, at no-cost to you, these tools and materials by mailing or faxing the Cardiovascular Resource Materials Order Form (PDF, 69 KB) to the Wisconsin Cardiovascular Health Program. The order form also includes website locations for guidelines and scientific statements that are referenced in the guidance tool.
