How to Electronically Order Forms
and Publications
The forms and publications requisition forms (F-80025A and B) are
fillable Microsoft Word documents. These are used to order forms and publications stored in
the Department's Forms and Publications Center. If you are
ordering 15
or less different forms or publications, use the F-80025A version. If you
are ordering more than 15 different forms or publications, use the F-80025B version.
Paper versions are not available for all forms and publications.
Before requesting a paper version of a form or publication, please review
the appropriate forms or publications list to ensure availability in paper
format. If the entry in the column "Other Location" reads
"None", paper versions
are not available. All other entry types will provide ordering
instructions.
Instructions
- Open the F-80025 A or B form, depending on the number of items you
are ordering.
Note: When you open them, you may see a dialog
box. If so, please do the following:
- Choose "Enable
Macros," and
- When asked if the document is from a trusted source, choose
"Yes."

Complete the form by tabbing through the input fields, NOT by using
the return/enter key.
- From the toolbar, choose File, Save As, give the document a name
and save it to a location on your computer that
you will remember.
- Close the saved document.
- After the form is completed, please e-mail it to the forms
manager. Please see the list below for who the
appropriate forms managers are.
Notes: Do not use the Send To function in Microsoft Word
to e-mail the request. Use the form prefix(es) to identify a manager
from the list below. If the order has more than one prefix,
please send it to the manager that is responsible
for the majority of the order as determined by the prefixes and this
manager will work with the others to complete your request.
- Once you have determined which forms manager to send the request
to from the list below, please choose the forms manager's e-mail hyperlink from the list
below.
- Attach the saved F-80025 A or B to the e-mail.
- Send the e-mail.
Please note:
- When placing your order, please do not order more than a three month supply of forms.
- Forms with a DES prefix (Division of Economic Support) are
no longer stocked in the DHS' forms center. Please contact the Department
of Workforce Development forms office at 608-266-8002 or 608-266-3283
for ordering information.
