EMT-Basic Intermediate Technician

EMT-Paramedic
Contact Information
EMS personnel licensed or trained in other states may
be eligible for licensure in Wisconsin. Wisconsin requires current
National Registry certification for licensure. Documentation and materials
needed to determine license eligibility are outlined below for each
license level. Electronic applications are submitted through Wisconsin
E-Licensing.
First Responder
- Complete an initial application in Wisconsin
E-Licensing.
- Complete the Verification of Out-Of-State
Education form and Verification of Out-Of-State Licensure (available
in Wisconsin E-Licensing),
print and submit to appropriate agencies for verification.
- Submit a copy of the Certificate of Completion for an approved
Terrorism Awareness course
- Submit a copy of you National Registry card
(applicants with current First Responder (or higher) National
Registry certification)
- Be prepared to submit:
- A copy of both sides of your current CPR for
Healthcare Professional
certification card
- A copy of the Certificate of Completion for your most recent First
Responder refresher course
Materials will be reviewed and applicants notified of eligibility.
Eligible applicants without current First Responder or higher National
Registry certification will need to pass the First Responder National
Registry examination. A letter of authorization for entry to
the exam will be sent to you upon proof of eligibility.
EMT-Basic
- Complete an initial application in Wisconsin
E-Licensing.
- Complete the Verification of Out-Of-State
Education form and Verification of Out-Of-State Licensure (available
in Wisconsin E-Licensing),
print and submit to appropriate agencies for verification.
- Submit a copy of the Certificate of Completion for an approved
Terrorism Awareness course
- Submit a copy of your National Registry card
(applicants with current Basic (or higher) National
Registry certification)
- Be prepared to submit:
- A copy of both sides of your current CPR for
Healthcare Professional
certification card
- A copy of the Certificate of Completion for your most recent
EMT-Basic refresher course

Eligible applicants without current EMT-Basic or higher National Registry
certification will need to pass the EMT-Basic National Registry

EMT-Intermediate Technician

Education form and Verification of Out-Of-State Licensure (available
in Wisconsin E-Licensing),

- Submit a copy of the Certificate of Completion for an approved
Terrorism Awareness course
- Submit a copy of you National Registry card
(applicants with current Intermediate-85 (or higher) National
Registry certification)
- Be prepared to submit:
- A copy of both sides of your current CPR for
Healthcare Professional
certification card
- A copy of the Certificate of Completion for your most recent
EMT-Basic and Intermediate Technician refresher courses
- EMT-Intermediate-85 training information including:
- A syllabus/course outline including the name and location of the
institution where training was received
- Dates of training
- Topic outline of all didactic and practical training
- Number of hours completed broken down by classroom didactic
training, in-hospital clinical experience and supervised field
experience
Materials will be reviewed and applicants notified of eligibility.
Eligible applicants without current EMT-Intermediate-85 or higher National
Registry certification will need to pass the EMT-Intermediate-85 National
Registry examination or successfully complete the Wisconsin
EMT-Intermediate Technician course and pass the Wisconsin EMT-Intermediate
Technician written and practical exams.
The National Registry EMT-Intermediate-85 exam is not administered in Wisconsin.
EMT-Intermediate
- Complete an initial application in Wisconsin
E-Licensing.
- Complete the Verification of Out-Of-State
Education form and Verification of Out-Of-State Licensure (available
in Wisconsin E-Licensing),
print and submit to appropriate agencies for verification.
- Submit a copy of the Certificate of Completion for an approved
Terrorism Awareness course
- Submit a copy of you National Registry card
(applicants with current Intermediate-99 (or higher) National
Registry certification)
- Be prepared to submit:
- A copy of both sides of your current CPR for
Healthcare Professional
and ACLS certification cards
- A copy of the Certificate of Completion for your most recent
EMT-Intermediate (I-99) refresher course
- A copy of you National Registry card
(applicants with current Intermediate 99 (or higher) National
Registry certification)
- EMT-Intermediate training (based on 1999
US DOT EMT-I national curriculum) information including:
- A syllabus/course outline including the name and location of the
institution where training was received
- Dates of training
- Topic outline of all didactic and practical training
- Number of hours completed broken down by classroom didactic
training, in-hospital clinical experience and supervised field
experience

(applicants with current Intermediate-99 (or higher) National
Registry certification)

Healthcare Professional

- EMT-Paramedic training
(1000 hours minimum) information including:
- A syllabus/course outline including the name and location of the
institution where training was received
- Dates of training
- Topic outline of all didactic and practical training
- Number of hours completed broken down by classroom didactic
training, in-hospital clinical experience and supervised field
experience
- Summary of all competencies achieved during
Paramedic
training
- If your EMT-Paramedic course was less than 1000 hours, you may also
want to submit the following for consideration to meet the 1000 hour
requirement:
- Verification of EMT-Paramedic work experience
- Certificates of Completion for EMT-Paramedic continuing
education courses
Materials will be reviewed and applicants notified of eligibility.
Eligible applicants without current EMT-Paramedic National Registry
certification will need to pass the EMT-Paramedic National Registry
examination. A letter of authorization for entry to the exam will be sent to you upon proof of eligibility.
