Eligibility Management: Wisconsin Funeral and Cemetery Aids Program

The Wisconsin Funeral and Cemetery Aids Program (WFCAP) is for funeral, cemetery, and crematory providers. It covers service provider expenses that aren’t fully paid for by an eligible decedent’s estate, family, or other resources.

In May 2022, the Wisconsin Department of Health Services (DHS) updated the WFCAP manual, application, other forms, and created a Payment Summary that is sent to the executor or family representative. We also released an informational brochure for families. Read the Service Provider Update (PDF) for details.

Just for service providers

We offer information to help service providers understand and access WFCAP funding. Service providers include staff at:

  • Funeral homes.
  • Cemeteries.
  • Crematories.

Eligibility

Service providers receive WFCAP payments directly. We offer up to $1,500 for unmet funeral expenses. We also offer up to $1,000 for unmet cemetery and crematory expenses for some decedents enrolled in any of these programs:

If a decedent wasn’t enrolled in one of these programs at the time of death, the decedent may still qualify for WFCAP benefits. The decedent may still qualify if they were eligible for Medicaid at the time of death or due to the cause of death. Before the service provider submits a WFCAP application, the decedent’s representative or family must apply for a posthumous decision. That will determine if the decedent was eligible for Medicaid. To request the application, the family should contact its Income Maintenance and Tribal Agency.
