Wisconsin Fatality Assessment and Control Evaluation (FACE) Program

The Fatality Assessment and Control Evaluation (FACE) Program operated in Wisconsin from 1995 - 2006.  It was designed to identify and study fatal occupational injuries.  To do this the FACE Program collected basic information about all fatalities in Wisconsin and conducted in-depth studies of fatalities that are: machine-related, highway work zone, or youth workers under the age of 18.  The Wisconsin FACE Program was funded by the National Institute for Occupational Safety and Health (NIOSH) (exit DHFS) and upheld the objectives set forth by NIOSH "to identify work situations at high risk for fatal injury and formulate and disseminate prevention strategies to those who can intervene in the workplace".  For more information about the Wisconsin FACE Program, please read our brochure (PDF, 59 KB) or frequently asked questions.
