Social and Community Services
Social and Community Services staff provides services to people
living at Central Wisconsin Center and within communities in the State of Wisconsin.
Services provided include Social Services to individuals living at Central Wisconsin
Center, and community services through Short Term Admission Services, Transition Services
and Community Capacity Team Services.
The Community Services staff arrange short-term admissions for community clients,
provide community consultation through the Capacity Team, and arrange for transition of
people living at Central Center to community living sites.
The Community Capacity Team provides assistance for follow up for persons who have
transitioned from Central Center to community living arrangements and for individuals
living in the community who are in need of consultation regarding behavior management or
program planning issues. The team is available for crisis intervention, program planning,
and person-centered planning.
The Physical Assessment Program serves clients with significant medical and therapy
needs. Average length of stay is fourteen days. Services include medical evaluation and
consultation on chronic care problems such as seizures, positioning, feeding, and
respiratory status. The Evaluation and Treatment Program is designed to serve medically
involved clients who may be admitted for six to twelve weeks to recover from surgery, or
receive intensive therapy and treatment. The goal of this program is to facilitate
positive changer in areas to improve health and functioning.
The Developmental Evaluation Clinic (DEC) provides diagnostic, evaluative and planning
services for community clients through a comprehensive, intensive interdisciplinary
evaluation. Clients are admitted for a five-day inpatient evaluation, which includes
medical, psychological, psychiatric, occupational therapy, physical therapy, audiologic,
speech, dietary, education, and vocational evaluations.
Social Work staff is the primary contact for parents and guardians of individuals
living at Central Wisconsin Center, keeping parents and guardians informed of changes in
health status and treatment programs. Social workers are the primary liaison between CWC
and county case managers, guardian ad litem attorneys, and community agencies serving
people living at CWC.
Community Service staff coordinate the transition of services for people living at
Central Wisconsin Center who move to community living arrangements. Transition staff
works with counties, families and guardians to identify individuals for transition and
assist in identifying services necessary to support each individual.
