Names Removed From Wisconsin Nurse Aide Registry

On May 7, 2005, the Wisconsin Department of Health and Family Services, Division of Quality Assurance completed an in-depth audit of nurse aides placed on the Wisconsin Nurse Aide Registry by reciprocity from other state's Registries.

The audit consisted of reviewing 1,220 Nurse Aide Registry Out-of-State Applications submitted to, and processed by, Promissor, Inc., from January 2003 to October 2004.

As a result of its review of these applications, the Department determined that a number of nurse aides needed to be removed from the Registry because they had not met the training and/or testing requirements necessary to be placed on the Wisconsin Nurse Aide Registry.

Accordingly, Promissor, Inc., informed these individuals that their names have been removed from the Wisconsin Nurse Aide Registry. These individuals are currently ineligible for employment as nurse aides to provide nursing related duties.

The following individuals have been removed from the Wisconsin Nurse Aide Registry due to inadequate training and/or testing procedures by their training or competency testing program(s).

These individuals are currently ineligible for employment as nurse aides to provide nursing related duties.
