Federal Caregiver Background Check Pilot Program - Hiring Data

General Information:

All providers affected by the pilot submit aggregate hiring data to DQA on a quarterly basis using the Quarterly Data Collection Tool.

The Quarterly Data Collection Tool is due 23 days after the end of the quarter.  This information is then calculated by county and by provider type and submitted to the Centers for Medicare and Medicaid Services (CMS).

January 2006 - March 2006 (Data for La Crosse is from February 1, 2006 - March 31, 2006; data for Dane, Kenosha and Shawano is from March 1, 2006 - March 31, 2006)
