2007 and 2008 Consumer Information Reports for Nursing Homes
and Facilities for People with Developmental Disabilities
The Consumer Information Report is produced annually by the
Division of
Quality Assurance for each Wisconsin nursing home and facility serving
persons with developmental disabilities (FDD - also known as ICF/MR).
The report provides information to aid residents or prospective
residents in assessing the quality of care provided by a facility.
Information includes facility specific federal violations and staffing
information.
Please note that there are a few facilities listed that do
not have reports. They are either state licensed only (i.e. not
accepting patients funded by Medicare or Medicaid) or else were
established too recently to generate reports. Also note that
facilities that ceased operations from 2007 to the present do not appear
on this report.
Below are links to alphabetical and county lists of Consumer Information Reports.
If you do not know what county a community is in, please see Wisconsin
Cities, Villages, Townships and Unincorporated Places
(PDF,
160 KB)
All documents are PDF files. Note also that blue text within the reports
indicate links that will take you to the
appropriate sections and appendices.

If you require further data on
these facilities, see "If You
Have Complaints about Health/Residential Care Providers." Please also review "Choosing
Wisconsin Residential Options" for information on a
county-by-county basis about other possibilities
for providing health or residential care.
We recommend that, in addition to reviewing these reports, you do the
following:
-
Visit the facilities you are interested in (consult
the various guides to choosing a nursing home via Wisconsin Nursing Homes
- this link also contains address directories).
-
View this link Nursing Home State Citations -- this contains
information on recent surveys that determined the existence of state code
(but not federal) deficiencies.
-
View the annual Wisconsin
Nursing Home and Facilities Serving People with Developmental
Disabilities site for basic information for both nursing
homes and FDDs.
-
View the federal Health Care Financing Administration's Nursing Home Compare site (see
link at bottom of page). This site contains more up-to-date information about survey
activities for individual nursing homes or other facility types.
